Job Overview:

Job Details:
- Providing individualized support in facilities and group homes
- Following the client’s treatment plan outlined and created by the client’s family and the case management team
- Basic understanding of smart phone applications, access to email communication, and awareness around processing electronic documents such as daily session notes
Requirements:
- DSP certification from an accredited school required
- 6-month prior experience in a long-term care setting required
- Active CPR certification
- Reliable transportation
- Compliant with facility requirements for immunizations, background checks, drug screenings, etc.
