Key Responsibilities:
  • Reports and documents patient/client s needs and responses to the appropriate Case Manager and/or Manager.  Reports patient/client s change in condition or situations immediately.
  • Adheres to patient/client LNA care plan as set up by the primary nurse while maintaining a healthy, safe environment.
  • Demonstrates competency in carrying out patient care/treatments.
  • Reinforces the teaching of the primary nurse by reviewing personal care techniques with caregivers and patients.
  • Integrates problem-solving methodology and quality improvement in approaching problem areas related to patient care and organizational processes.
  • Completes documentation in clinical records, timesheets.
  • Attends a minimum of 12 hours of in-services per year for NH, 48 hours every 1 years in MA
  • Adheres to MAS safety, organizational and ethical standards.
  • Adheres to MAS policies, protocols, and procedures, including infection control standards.
  • Ensures confidentiality of employee, legal, client/patient, budget and all company matters
  • Provides care while promoting client/patient dignity and provides a calm, supportive and therapeutic environment for client/patients and their families.
  • Acts as a resource to others (students, peers, and other departments) in the provision of client/patient care.
  • Demonstrates commitment to customer service.
  • Positively reflects the agency within MAS the organization and the community.

Requirements:
  • Must have an active license in State of Employment
  • Certification from an accredited school required
  • 1-2 years prior experience
