Position Summary:
Delivers home health care to clients/patients of all ages and families whose needs range from basic to complex and are rapidly changing by utilizing the nursing process.  Creates a therapeutic environment while identifying and treating human responses to actual or potential health problems through such services as case finding, health teaching, health counseling, and the provision of care supportive to or restorative of life and well-being. 
Position Requirements:
  • Demonstrates knowledge and skills in assessing needs, planning care, carrying out interventions, and evaluating outcomes and responses in relation to the ages of the client/patient population served.
  • Ability to plan and perform diversified duties requiring an extensive knowledge of a particular field, and the use of a wide range of procedures.  Demonstrates particular knowledge about a specific subject(s) or client/patient population for which he/she is a resource.  Involves the exercise of judgment in the analysis of facts and conditions regarding individual problems or transactions to determine what actions should be taken, within the limits of standard practice.
  • Ability to perform job alone following standard practices and any of many procedures.  Receives training and guidelines from supervisor on non-routine duties and determines action to be taken in handling all but unusual cases.
  • Direct caregivers and certain others, involved in working with critical patient care equipment, sterile pharmaceutical preparation or food preparation, may not wear artificial nails.
